VIRTUAL CONSULT
To keep your health and safety in mind, we have set up a virtual consult program. You now can get all your orthodontic questions answered from the comfort of your home, free of charge.
What orthodontic treatment is right for me? What is the average time it will take? Am I a candidate for Invisalign? How much will this cost? What are my financial options?
These questions, and any others you may have, we can answer without an office visit! Let us show you the Cusimano difference without any obligation, contract or fees.
Step 1.
Fill out the Virtual Consultation Form.
Step 2.
Email consultation photos (see instructions below) which we need to best evaluate your *case. Please send photos to info@cusimanoorthodontics.com.
Step 3.
After Dr. Cusimano has reviewed your records, our Treatment Coordinator will contact you to discuss your treatment plan, obtain insurance benefit information, discuss treatment fee payment options, and design a custom payment plan that is interest fee.
Step 4.
We will then set up a consultation in our office so you can meet Dr. Cusimano, tour the office and we can take your complimentary records to start you on your orthodontic journey at Chris Cusimano Orthodontics!
*Until x-rays have been taken the treatment plan provided is only an estimation of treatment based on the records you have provided our office. Once the proper records have been taken and an examination performed, we can confirm the treatment plan.

Email is a very popular and convenient way to communicate for a lot of people, so in their latest modification to the HIPAA act, the federal government provided guidance on email and HIPAA.
The guidelines state that if a patient has been made aware of the risks of unencrypted email, and that same patient provides consent to receive health information via email, then a health entity may send that patient personal medical information via unencrypted email.
